The following are example documents that can be used for each part of the process.

Hey Team

As I mentioned at standup on Monday, we're setting up for our performance reviews again. Yay! This will follow the same format as before, and consists of three parts:
- Fill in a Google form where you provide feedback on your peers. I'll send you each a separate email so you know exactly who to provide feedback for.
- Write your own self-assessment based on the set of questions I also send you via email.
- We then have a scheduled chat about your self-assessments and compare that to the feedback your team has provided for you. This session will be with you, [Department Head], [HR] and me only. Where there is correlation, we acknowledge that you and the team see your role the same way. Where there are differences, we will discuss those in more detail to unpack why they may exist.

Each feedback session will be between 1 and 2 hours, and we'll begin those in early May. There are a lot of us now, so it will take a few weeks to get through everyone. I'll also send a calendar invite to each of you so you know exactly when your review is.

You are required to complete your evaluation of your peers by [DATE] please.

A reminder that the feedback you provide will be treated as confidential. This is meant to be a way to provide constructive feedback, but do still be honest.

Combined Self Assessment and Peer Review Email

Annual Reviews

Here is everything you need to know about the upcoming reviews. We use a peer review system, where reviews are provided by the people you work with and you get to provide reviews for them too. You also need to do a self review. We then meet and discuss the two sets of reviews and compare notes.

**Your Self Review**
Your review is scheduled for [DATE]. Here is the list of talking points for you to complete before we meet:

- 5 self-praise items
- 5 self-improvement items
- 5 ways other team members already help you
- 5 ways other team members can help you more
- 5 ways I feel I help other team members

Feel free to be as light or in-depth as you like. This is really a kick-off point for conversation so don't feel as though you need to write a massive essay for each point, but you can if that's your preferred style. Bullet points are absolutely fine too. Please type out your points in whatever way you like, but you don't need to send them to me before we meet. We will go through your list and the feedback from the team in detail during the session itself. I will need a copy of your self review afterwards so we have a point of comparison for future reviews.

You should probably expect to spend 2-3 hours on this self-review in total. As the reviews are a few weeks away, you can fit this in easily.

If you want to, you may choose one additional person not on this list to provide a review for, but this is completely optional.

These reviews are confidential and I will be the only person who can see who has written what. I take this portion of the review process very seriously, and will only escalate issues for a detailed discussion with the Board where there is something very concerning that needs to be dealt with.
You will receive a separate form link from [HR] to provide a review of me, so that the same level of confidentiality is maintained for all of us.

The ultimate goal of this process is to provide a balanced and rounded view of your performance from the people you work with most of the time. We aim to be supportive and provide constructive and encouraging feedback, so that we can all improve in the right areas, but also know where we're doing things right and continue to do so.

Yes this is a big time investment, but it’s worth it. Put the effort in for both yourself and your team. You can probably spend 20-60 minutes on each person, depending on how closely you’ve worked with them and how much you want to write about them. You have about two weeks, which should be plenty of time.

I send this via email to each person in preparation for their review, usually a week or two before the date.

Google Form Setup

Set up a Google Form for the team to provide their feedback on. The data is collected in a spreadsheet, which makes it easy to sort and filter for each person.
